Circulatory Technology Inc is featuring the Better Bladder, a compliant venous reservoir placed between the patient and the pump inlet during ECMO or CPS. The Better-Bladder is available with either heparin or non heparin coating, in 1/4

With a roller pump, the Better-Bladder:

1. Takes full advantage of the computer controlled roller pumps allowing pump speed to be controlled as a function of pump inlet pressure.

2. Allows use of augmented venous drainage to facilitate venous blood withdrawal. This eliminates the need to place the bladder on the floor and allows its placement above the pump thereby reducing tubing length and prime volume. Actively pulling the blood also allows the use of a smaller diameter cannula (e.g. when percutaneous cannulation is used) where the additional cannula resistance to flow is overcome by a slight negative pressure provided by the pump. Using the Bigger Better-Bladder (BBB38) and assisted venous drainage allows the replacement of the 1/2

 

3. Eliminating the bladder placement at floor level:

a. Simplifies transporting the patient.

b. Makes it easier to reach a bladder that is at eye level than below knee level.

c. Reduces sterility issues of blood ports near the floor/feet and allows mopping along the ECMO pump.

d. Eliminates the need for raising the bed with blocks, the patient with mattresses, or the need for a special bed.

 

4. Provides better flow dynamics than the silicone bladder resulting in lower stasis and clotting.

5. Inlet and outlet tubing made of standard PVC that does not kink as easily as the tubing coming off the silicone bladder.

 

6. Requires very little volume change to cause a change in pressure (i.e. to control pump flow as a function of pump inlet pressure). The large volume change associated with the silicone bladder can cause hemodynamic instabilities.

 

When using centrifugal pumps, the Better-Bladder provides:

1. Compliance between the patient and the pump inlet that absorbs the very high negative pressure spikes associated with sudden stoppage of venous flow (e.g. chattering). Reduction in high negative pressure reduces the damaging force applied at the tip of the venous cannula.

 

2. Traps large air bubbles before they reach the centrifugal pump. Centrifugal pumps act as a blender that chops large bubbles into microbubbles that are difficult to remove. Trapping air bubbles reduces microbubbles reaching the patient.

3. Allows non invasive pressure measurement at the pump inlet. This eliminates the need for a stopcock that, if opened accidentally, introduces air into the venous line.
